    Certificate" button below).                             
  2. Contact ELEMENTS Spa and Salon at (217) 546-0740 to receive a gift
    certificate number.  Write this number, along with the amount, your name,
    recipient's name, your greeting, and the purchase date on the gift
    certificate form.
  3. A copy of this certificate is on file with ELEMENTS Spa and Salon.
  4. Certificate will expire 1 year from the date purchased.
